*A copy of  birth certificate for all NEW registrations is required to be shown to Registrar in order to verify birth date.
*Members transferring into Wembley boundaries or joining for the season due to "no team" must ensure they have completed appropriate forms available (ie. parent declaration or release forms - see  section WMHA Info\Forms)

**This includes a Fundraising Fee of $200 plus a Hockey Alberta Insurance Fee (U7-U9 $65 and U11-U18 $75)
